The following forms are available for use:

FHHS Library borrower registration & PIN Number.  Print this form, complete the details and send it in to the Library in order to register as a borrower and/or to electronically request photocopies from other libraries

Request for loans of books, journals and videos from FHHS Library only - do NOT use this form if you want to request copies from other libraries.  See below for that form.

Recommend new material for the Library Collection

Feedback.  Do you have any comments on our services or collection in general or on any specific aspect.  Do you find the internet site useful, or can you suggest changes which might make it more useful/useable.

The forms listed below are for services that will be conducted only for work-related purposes.  If you are studying at university, please use the Library staff and resources at your university to assist you with your non-work related searches.

Request for loans from other libraries

Request for photocopies from other libraries (PIN required for this service, use FHHS Library borrower registration & PIN Number form to request one). Please observe the Copyright Act in all copying you request from print or electronic sources.  For more information see the Australian Copyright Council's Information Sheets:  An Introduction to Copyright in AustraliaFair Dealing, Copying for Research or Study, & Libraries, (Non-Profit): Introduction to Copyright. Fair dealing provisions in most instances restrict us to providing you with one article from a journal issue or one chapter from a book.

Literature Search Request Form 

Literature Search Evaluation form (your feedback to us on the usefulness or otherwise of information we have provided to you in a search)

Form to request a copyright declaration PIN.  You need to complete this form once in order to electronically request photocopies from other libraries.  The form, which you must print, personally sign and return to the library, is held as an electronic signature for purposes of declaring your adherence to copyright laws.  The PIN then eliminates the need for a personal printed signature for each item requested.
